The Wroclaw Medica Center pays particular attention to respecting your privacy and that of other users visiting the WMC website. The site does not collect any information automatically, except for the information contained in cookies.

What are cookies

Exercise of rights concerning personal data processed by wroclaw medical center

As we are in possession of your data you have rights in relation to its processing under the provisions of the General Data Protection Regulation (RODO). Below you will find all the necessary information regarding the procedure for making and implementing requests for the exercise of your rights.

You have the following rights in relation to the processing of your personal data:

  1. the right to object to the processing of data for marketing purposes
  2. The right to object to the processing on the grounds of your particular situation - in cases where we process your data on the basis of our legitimate interest,
  3. the right to withdraw consent,
  4. The right of access to personal data,
  5. The right to request rectification and completion of personal data,
  6. the right to request the erasure of personal data - only if we are not obliged by law to process it,
  7. The right to request the restriction of the processing of personal data,

 

A response in accordance with the scope of the request will be given without undue delay, but no later than 30 days from the date of notification. In justified cases, i.e. due to the complexity of the request, we may extend this period while informing you of the reason for the delay.

 

The exercise of data subjects' rights is free of charge. However, in the case of the right of access, we reserve the right, in accordance with the law, to charge a fee for subsequent copies of the data resulting from the exercise of the right under Article 15 RODO.

 

The Wrocław Medica Center may impose restrictions on the exercise of the rights of persons whose data it processes and deem the requests to be unjustified, in particular by referring to the provisions of, inter alia, the Act on Health Care Activities, the Act on Patient's Rights and Patient's Ombudsman, and where the exercise of the right may adversely affect the rights and freedoms of other persons (data subjects).

 

Information on personal data processed in the video surveillance system

In accordance with Article 13 of the General Data Protection Regulation of 27 April 2016. (Official Journal of the EU L 119 of 04.05.2016), we would like to inform you that.

  • monitoring is used to ensure the safety of persons, the protection of property and legally protected secrets
  • the basis for processing is Article 6(1)(e) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016;
  • monitoring records will be stored for a period of 11 days and are automatically overwritten
  • the recipients of the personal data will only be the entities entitled to receive them on the basis of the provisions of the applicable law
  • the person recorded by the monitoring system has the right to request access to, rectification, erasure or restriction of processing of personal data concerning the data subject and the right to object to the processing, as well as the right to data portability;
  • monitoring data will not be used to make automated decisions, will not be used for profiling purposes
  • a person recorded by a monitoring system shall have the right to lodge a complaint with a supervisory authority

Your rights in relation to the processing of your personal data

You have the following rights in relation to the processing of your personal data:

  1. the right of access to your personal data (i.e. information on how we process your personal data and to obtain a copy of your data)
  2. the right to request the rectification (i.e. amendment) of your personal data
  3. the right to have your personal data deleted, only if we are not obliged by law to process them
  4. the right to request the restriction of the processing of your personal data
  5. the right to portability of your personal data, i.e. the right to receive your personal data from us
  6. the right to object to the processing of your data for marketing purposes and on grounds relating to your particular situation - in cases where we are processing your data on the basis of our legitimate interests

Purposes of processing your personal data and legal basis for processing

 We will process your personal data in order to:

  • cover you with a medical care contract,
  • make business settlements,
  • handle complaints, if received,
  • provide materials to promote the controller's own products and services,
  • archive your data in accordance with the law,
  • ensure the safety of persons and property (video surveillance in place).

The legal bases for processing your data are:

  • medical coverage agreement,
  • the legitimate interest of direct marketing of the controller's own services and the assertion of claims, as well as ensuring the security of persons and property,
  • legislation.

Retention period of your personal data

We will keep the data that we process under a healthcare contract for the purpose of providing medical services for the period specified by the healthcare contract, by law, and in the legitimate interests of the data controller (marketing) until you object.

Recipients of your personal data

 We will transfer your personal data:

  • to entities processing data on behalf of the controller,
  • entities authorised by law,
  • to entities authorised by you.

Right to lodge a complaint with an authority

You also have the right to lodge a complaint with the supervisory authority in charge of personal data protection, i.e. the President of the Data Protection Authority.

Information on profiling

We do not carry out profiling on the basis of your personal data, i.e. automatic assessments of certain personal factors about you will not be undertaken.

Obligation to provide data

The receipt of personal data was dictated by a condition of coverage under the healthcare contract and a statutory requirement.
